Ambulatory and Emergency Appointments (B.W. Furlong & Associates)

Scheduling an ambulatory appointment:
Please call 908-439-2821 to schedule an ambulatory (on-farm) appointment. Please have a couple dates and time frames in mind when you call. Based on the nature of our ambulatory appointments, we schedule appointments in 2-hour intervals. Our veterinarian or office staff will contact you when they are on their way.

Scheduling an emergency appointment during business hours: Please contact our office.
Emergency appointment after business hours: Please contact our office. Your call will be answered by our answering service who will direct your call to our veterinarian on-call.
How to prepare: Please make sure that your horse is in his/her stall and moderately groomed prior to the veterinarian’s arrival. For the safety and health of our veterinarians and technicians, please follow our COVID-19 policies.
Getting in touch with the veterinarian: Please contact our front office to get in touch with any of our veterinarians. If they are not available at the time of your call, they will get a message to them and the veterinarian will give you a call back.

Ambulatory Appointments
(Peak Performance Equine Services)

New clients: If you are a new client of Peak Performance, please contact our office at 908-439-9104 to set up your account prior to scheduling an appointment. Once your account has been set up, our client relations team will put you in touch with our veterinarians to schedule an appointment with one of our Peak Performance veterinarians. Please have any pertinent past medical history readily available for your appointment.

How to prepare: Please make sure that your horse is in his/her stall and moderately groomed prior to the veterinarian’s arrival. For the safety and health of our veterinarians and technicians, please follow our COVID-19 policies.

Elective Hospital Appointments (B.W. Furlong & Associates) 101 Homestead Road, Oldwick, NJ 08858

Scheduling an appointment: Please call 908-439-2821 to schedule an in-clinic appointment. Please have a couple dates and times in mind when you call. Authorization forms will be sent to your email via Docusign for your digital completion prior to your scheduled appointment.
Patient updates: All updates from the treating veterinarian will take place over the phone.
Cancellation Policy: There will be a $52 cancellation fee assessed for any appointments canceled with less than 24 hours’ notice.

Emergency Hospital Referrals

Drop off and pick up: Please call our office (908-439-2821) when you arrive for your scheduled appointment and follow our temporary COVID-19 policies.
Visitation: Due to the COVID-19 pandemic, we are not allowing visitation at this time. We understand that emergencies are stressful for clients, so we are committed to keeping you well informed about your horse's condition while they are in our care.
Authorization Forms: Authorization forms for treatment will be sent to your email via Docusign for your digital completion prior to your scheduled appointment.
Deposits: A deposit is required for any hospital admission. This deposit will be applied to the total invoice.
Invoice Updates: Clients with patients in our clinic will receive regular updates on their invoice total throughout their stay. Additional deposits may be requested for the duration of their stay.

Payment Policies for Ambulatory and Hospital Appointments B.W. Furlong & Associates and Peak Performance Equine Services

We accept cash, check, credit card (by phone and through our website), Care Credit, and Scratch Pay.

Ambulatory: Monthly statements are mailed and/or emailed on the 1st of each month showing the previous month’s services, and payment is due by the 15th of the month.
Emergency: Payment is required in full at the time of service.
Hospital: Payment of in-clinic services is required in full at the time of discharge.

Scheduling an appointment for Equine MRI of NJ: Please call 908-439-2821 to schedule an appointment. Once confirmed, authorization forms for treatment will be sent to your email via Docusign for your digital completion prior to your scheduled appointment.
Scheduling an appointment for AEIW: Please call our office at 561-753-3226 to schedule an appointment. Once confirmed, authorization forms for treatment will be sent to your email via Docusign for your digital completion prior to your scheduled appointment.

How to prepare for an MRI at either location:
● Please make sure your digital paperwork is completed.
● The patient’s shoes will be removed for the imaging (front shoes for front limb scanning, hind shoes for hind limb scanning). If glue-on shoes are present, we kindly ask that you have them removed prior to arrival at our facility.
● We provide T&A hay for your horse; if you would like something different please bring with you at drop off.

Drop off and pick up at Equine MRI of NJ:
Please call our office at (908) 439-2821 upon arrival and follow our temporary COVID-19 protocols.
Drop off and pick up at AEIW:
Drop off at 8am. Pick up time is patient-dependent, will we give you a call when your horse is ready to go home.

● We are now allowing “curbside check in” only.
● A technician will meet and escort your vehicle through our entry gate to retrieve your horse.
● The hospital and office are restricted to AEIW Personnel Only. No clients will be
permitted entry.
● If you wish to remain on site at any time during your appointment, please remain in your vehicle.

Payment Policy for both locations: Payment is required in full at the time of discharge. We only accept credit cards and wire transfers.

Furlong’s Soundness Center 1 Hill and Dale Road Lebanon, NJ 08833

Scheduling an appointment:
● Please contact the main office at 908-439-2821 for stall availability for in house services at FSC.
● If there are no stalls available at the time of inquiry, you may request to be added to the wait list.
● Please contact the main office to leave a message for an FSC team member for inquiries or scheduling requests for ship-in services with the Water Treadmill or Laser.
● Once availability is confirmed, the front office will forward any admission
paperwork/release forms for completion. Once completed and returned, drop off may
be scheduled.
Drop off and Pick up:
● Drop off/Pick Up Hours: Monday through Friday between 9 a.m. and 3 p.m.
○ Alternate arrangements may be made in advance by special request for
admission or departure outside of the standard hours noted above.

Trailers:

  • Any tractor trailer-sized rig must plan to drop off/pick up via the main hospital, located at: 101 Homestead Rd. Oldwick, NJ
  • All other size rigs may come directly to Furlong’s Soundness Center, located at: 1 Hill and Dale Rd. Lebanon, NJ
  • There is not a pull around driveway available at FSC. Shippers must be comfortable backing up to get in and out of the property. There is a large level driveway with ample room to turn around.

Shipping Paperwork:
Admission requires:
■ Current negative Coggins, within 12 months
■ Health Certificate within 30 days for out of state horses
■ Vaccine history, within 12 months (included on admission paperwork)
■ Horse residence and competition location history for the last 30 days
(included on admission paperwork)

Departure: If you require any shipping paperwork (Health Certificate, Coggins,
Vaccine Certificate) please notify the front office during scheduling, and provide
any necessary information (shipper and contact, destination address) at least 1
full business day prior to departure.

On Arrival:

  • An out call of ~30 minutes is always appreciated to ensure staff are available to admit the shipper to the property. Contact the main hospital at 908-439-2821; if outside of regular office hours, please leave a message with the On Call Answering Service.
  • Both properties have locked gates, please wait for staff to let you in.

Patient updates:

  • Upon admission, clients are generally notified of arrival and how the horse settles in.
  • A detailed Admission Report can be expected by email in the first week from Dr. Sarah Gold.
  • A detailed Progress Report can be expected by email about every 30 days with updates from Dr. Sarah Gold.
  • Due to the time-intensive nature of the hands-on patient care at FSC, updates are most often sent via email on a weekly basis from an FSC Team Member.
  • Clients may email anytime, or call in via the main office during regular business hours to leave a message for an update request or with any questions or concerns. Calls are returned in a timely manner, around patient care.
  • In the event of an emergency, you will be contacted via the best mode of communication as indicated on your admission forms.
  • Accurate and detailed admission forms allow for the FSC team to provide optimal
    communication and care. Please make sure all contact information on your paperwork and account is current.

Visitation: Due to the COVID-19 pandemic, we are not allowing visitation at this time.

Payment Terms:
Payments terms are as follows:
1. Deposit upon arrival - first month FSC Hospitalization Board and Dynamic Rehabilitation fee
2. The full monthly balance on the 15th of every month thereafter;
3. Remaining balance at time of discharge

Contact the office for FSC Hospitalization Board and Dynamic Rehabilitation fees. Please note, pricing may vary based on the needs of each patient as each has a customized treatment plan. There may be other necessary care, medications, supplements, and/or grooming recommendations while patient is receiving treatment that is at additional charge to the client (examples - dental float, Spring/Fall vaccines, farriery, chiropractic, additional diagnostics [radiographs, MRI, bloodwork, fecals etc.])

Scheduling an appointment: Please call 908-439-2821 to schedule an appointment. Please have a couple dates and times in mind when you call. Authorization forms for treatment will be sent to your email via Docusign for your digital completion prior to your scheduled appointment.

Drop off and pick up: Please call our office (908-439-2821) when you arrive for your scheduled appointment and follow our temporary COVID-19 policies.

Patient updates: All updates from the treating veterinarian will take place over the phone.

Visitation: Due to the COVID-19 pandemic, we are not allowing visitation at this time for your safety as well as the safety of our staff. Our veterinarians will keep you well informed of your horse's condition and can arrange a video conference if necessary.

Deposits: A deposit is required for any scheduled surgery. This deposit will be applied to the total invoice.

Cancellation Policy: There will be a $52 cancellation fee assessed for any appointments canceled with less than 24 hours’ notice.
